To become a patient on our service, please fill out and return our patient packet via fax or email. We take most insurances, but please call us first to confirm. Once we’ve received your information, a member of our staff will contact you to set a time for your initial visit. This process is no different than if you were going to visit a new Provider, except our Providers will benefit from the ability to learn about you in your living environment. During this visit, we will assess your current mental and physical state, medical history, medical condition, medications, environment, support structure, mobility, family dynamic, obstacles, and the short and long terms goals of everyone involved. From this visit, we can design a plan of care to be shared with you and the entire care-team. Once on our service, we can schedule visits in advance or we will reach out to you, your family, or facility staff to let everyone know when we will be visiting your facility or neighborhood. Our patients reside in independent living, assisted living, memory care, personal care homes, and private homes.
The geriatric population requires an exceptional level of care coordination as they typically have multiple chronic conditions and receive services from many providers. Over the last 48 months, we’ve encountered dozens of great home health and hospice agencies who share our philosophy and are happy to provide you with these experiences as you choose your care team.
