The Lady with the Lamp - Visiting Nurse Practitioner

The Lady with the Lamp is a nurse practitioner practice that brings healthcare services to home-bound geriatric patients in their place of residence, including personal homes, independent-living, assisted-living, skilled-nursing facilities, and personal care homes. We provide meaningful care and coordination for our patients to increase their quality of life, prevent unnecessary hospitalizations, and allow them to age in-place. Our nurse practitioners and staff are well-versed in the chronic conditions facing the geriatric population. We leverage this expertise to manage the comprehensive care needs of our patients through proactive communication with the entire care team, including the patient, their families, facility staff, home-health companies, and hospice agencies.
